Video means that the little view-piece of this item [he pulled out his own watch and gestured at its display, which showed what the device's camera could see] you see here shows a real-time feed of the person on the other side. It's like talking to someone face-to-face, but you can be out hunting in the forest while the person on the other end is walking through the library or enjoying the view from the clock tower. You just have to keep the watch pointed at whatever you want them to see.

Voice-only means they don't see anything, but they hear whatever the pocket-watch picks up on your end – what you say, your breathing if it's loud enough, other voices, the like. Most people opt for it if it's the middle of the night or they're on the move.
